Book Report Forms

Book reports are done on independently selected books.  Students are free to choose any book on the accompanying list of books already approved by the Central Kitsap School District (CKSD); a book from the list of those approved by their English teacher and requiring written parent permission; and any book for which the student has obtained written permission from both parent and teacher.

The list of books already approved as supplemental materials by the CKSD:

These are the forms to be used for book reports.  They are posted as PDF files so you don't need to be running Microsoft Word to use them.  You can print any of these that you need.
